WAIVER AND RELEASE OF LIABILITY

I, the undersigned, acknowledge that participation in basketball activities involves inherent risks including, but not limited to, the risk of injury. I voluntarily assume full responsibility for any risks or loss, property damage, or personal injury that may result from participation in this event.

I hereby release and discharge the event organizers, coaches, volunteers, and the Bayview Hill Community Centre from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may be sustained while participating in the Elevate Girls Basketball Exhibition Day.

In case of emergency, I give permission for event organizers to provide or seek medical assistance as necessary.

PHOTO/VIDEO CONSENT
I grant permission for photos/videos taken during the event to be used for promotional purposes by Elevate Girls Basketball.
